If an infant requires more than a few minutes of CPAP or PPV with a face mask or LMA, the team should consider an orogastric (OG) tube. Gas during CPAP and PPV travels through both the trachea and the esophagus. Air flow through the esophagus can fill the stomach, which may then push up on the diaphragm and impede effective ventilation.
Placing an uncapped 8F OG tube allows air to vent out of the stomach. Correct tube length can be measured from the bridge of the nose to the earlobe and from the earlobe to a spot located midway between the umbilicus and the xiphoid process.
