Ongoing Reassessment

Ongoing reassessment evaluates the patient’s response to the interventions the PALS provider has performed. After each intervention, the provider ascertains whether there is deterioration, improvement, or no change in the patient’s condition. Continued reassessment detects new problems as well. 

Ongoing reassessments guide the clinician on what to do next for the patient. Frequent reassessment should include all steps in the systematic approach to the seriously ill child, including:

  • The Patient Assessment Triangle
  • The Primary Assessment (ABCDE) 
  • An evaluation of abnormal anatomic and physiologic findings
  • An evaluation of the effectiveness of treatments performed
