Megacode at a Glance
- A 39-week newborn is delivered flaccid and apneic with a heart rate of 56 bpm (normal ≥100 bpm).
- Despite PPV at 40–60 breaths/min and MR. SOPA adjustments, HR remains low.
- Chest compressions are performed at 90 per minute with a 3:1 compression-to-ventilation ratio for 60 seconds before reassessment.
- When the heart rate remains 56 bpm, compressions and PPV continue until it rises to 80 bpm, at which point compressions stop and PPV is resumed at 40–60 breaths/min.
- Once the heart rate exceeds 100 bpm, the team transitions to postresuscitation care.
