Experience
Before joining Salud Family Health Centers as a Physician Assistant in 2019, she served there for two years prior with Community Healthcorps, a branch of Americorps.
As part of the transitions of care team, she visited over 300 hospitalized patients, aiming to reduce 30-day hospital readmission rates by almost 5% in one year. She also conducted over 25 home visits with high-risk patients for medication reconciliations, mental health screenings, and home-safety assessments. These tasks helped improve access to community resources via follow-up calls, scheduling appointments with a primary care provider upon discharge, and translating medical information from English to Spanish for more than 70% of patients.
Stephanie enjoys teaching and mentoring and has served as a preceptor to students enrolled in multiple disciplines of medicine. To stay current in the latest medical trends and best practices, she is a member of AAPA and CAPA. She also attends bi-weekly meetings to discuss the latest scholarly studies and articles to enrich her practice. These include topics such as journal review, clinical pharmacy, COVID-19 updates, and quality metrics.
Stephanie is committed to providing quality medical care, increasing access for the underserved, and improving patient outcomes. In addition, she is an advocate for universal healthcare reform.
