- In the case of a suspected overdose, clinicians should remember ABCDE (Airway Breathing Circulation Disability Exposure) when performing their initial evaluation.
- First, check the airway, looking for a pulse. Non-responsive patients should be intubated as soon as possible unless Narcan is otherwise indicated.
- Second, assess the patient’s breathing. If they’re intubated, ventilate appropriately, and if they’re not ventilated, put them on high-flow oxygen.
- Third, assess the patient’s circulation, checking the pulse, blood pressure, and rhythm.
- Fourth, assess the patient’s mental status. If it’s disabled, check for blood glucose levels and evidence of seizure activity.
- Lastly, evaluate for ongoing exposures, such as fentanyl patches.
